1. This service qualifies for your initial employee physical/medical statement/health evaluation for Colorado Child Care Licensing. 2. You are scheduling a precautionary phone call appointment just in case we need to contact you. If you indicate a medical condition on your medical history form, we will give you a call. Should you have no medical condition that affects your ability to safely perform your job, we will not contact you by phone. Instead, we will assess your submitted medical history form and send your medical statement by email following your precautionary phone appointment. 3. You will receive your form by email within 24 hours following your precautionary appointment. 4. If you are purchasing the service for multiple employees, stop here. It is easier if you make a bulk purchase. We will email you a promo code after your bulk purchase and you simply hand your employee the promo code to book their own appointment.
